Fill out the form. If you choose to submit the form, you will be counted as one of our 2012 members unless you tell us otherwise. Please do not submit the form unless you plan on sending payment and picking up your shares - you might be taking away a spot that someone else wants!

You do not have to print the form, but please use your email address in the subject line of the check so we can easily identify your payment.

If this form is still up, we have shares available. If you need an alternative payment plan, just let us know. We do require full payment by June 15, 2012 but understand the need to be flexible in these rough economic times.
